The position assists families in accessing appropriate services through collaboration with other agencies, community partners, and service providers, while also offering advocacy and family-centered support as needed. Work is performed in a combination of office, client home, and community settings and requires periodic after-hours, on-call services. Major responsibilities include assessing the range of services needed through interviews, home visits, collateral contacts, and other information-gathering methods.
